W3TC can do a lot of different caching. I believe you need to explicitly turn on "Browser Caching" and have it version and cache resources for longer than 30 days. You should also check your CDN and make sure it isn't set to override caching with an explicit 5 day cache.

It also looks like there are a few resources that you may not have control over, I'd tackle the CDN ones first.
